Varje match placerar en spelare som Mördare och upp till fyra spelare som Överlevande på en proceduralt genererad karta. Överlevande måste reparera fem generatorer utspridda över kartan för att aktivera utgångsportarna och fly innan de blir fångade. Mördaren måste hitta, fälla och hänga varje Överlevande som ett offer till Entiteten, den uråldriga kraften som styr riket där alla matcher äger rum.
Varje Mördare har en distinkt kraft som definierar hur de jagar. Fällan lägger ut björnfällor över kartan. Systern fasar genom hinder och teleporterar korta avstånd. Hillbilly använder en motorsåg för snabb förflyttning och omedelbara nedtagningar vid kontakt. Varje Överlevande bär en unik personlig fördel tillsammans med ett lastsystem som låter spelare bygga anpassade fördelar från de som låsts upp över hela roster.
