The premise is straightforward: humanity has gone extinct, and the machines that once depended on human blood are now running dry. V1, a combat machine, descends into Hell to find blood among the demons and souls trapped there. The structure follows the nine circles of Hell, moving across a Prelude and three Acts, with each layer ending in a boss encounter. The game draws on the framework of Dante's Inferno as a backdrop for its escalating design.

The core mechanic that sets ULTRAKILL apart is blood-based healing. Players cannot regenerate health passively. Dealing damage to enemies releases blood into the environment, and moving in close to absorb it restores health. This forces players to stay aggressive and maintain proximity to enemies rather than holding back at range.

Combat is measured through a real-time style scoring system that rewards variety, speed, and creativity. Repeating the same attack, standing still, or playing passively drops the score. Players are pushed to chain different weapons, movement options, and techniques together to maintain high combo ratings. The arsenal covers several distinct weapons, each with multiple unlockable variations that change how they function entirely.
