Geregisseerd door Morio Kishimoto, markeert het spel de eerste keer dat de Sonic-serie een volledige open-wereldstructuur heeft aangenomen, waarmee de traditionele afhankelijkheid van lineaire levelontwerpen wordt beëindigd.
Het verhaal volgt Sonic, die gescheiden raakt van Tails, Amy, en Knuckles nadat ze door een wormgat zijn getrokken. Gestrand op de Starfall-eilanden, een oude locatie overspoeld door mysterieuze robotachtige wezens, wordt Sonic geleid door een onzichtbare stem terwijl hij zoekt naar de vermiste Chaos Emeralds en een manier om zijn vrienden te redden en de geheimen van een beschaving die eerder kwam te onthullen.
