Quest Requirements

Before starting the Biohazard quest, you need to complete several prerequisites and gather specific items. Missing any of these requirements will prevent you from beginning or progressing through the quest.

The pigeon cage can be obtained from Jerico's house in Seers' Village. Priest gowns are found in various locations, including the Varrock Museum or by trading with other players. Gas masks are available from several NPCs or through the Grand Exchange.
Once you have met all these requirements, talk to Elena in her house in East Ardougne to start the quest. Elena's house is located in the residential area, and she'll provide the initial briefing that sets the quest in motion.

Step-by-Step Guide

The Biohazard quest involves multiple locations and requires careful attention to detail. Follow these steps exactly to complete the quest successfully.
1. Starting the Quest

Begin by talking to Elena in her house in East Ardougne. She's located directly west of the log balance shortcut. Elena will tell you about her father's friend Jerico, who maintains communication with West Ardougne through pigeons.
2. Getting the Pigeon Cage and Bird Feed

Head to Jerico's house, located just south of the northern bank in East Ardougne. Talk to Jerico first, then search the cupboard on the north wall to obtain bird feed. Go behind his house to collect a pigeon cage from the ground.
3. Creating a Distraction

Travel to the watchtower area near the wall between East and West Ardougne. Find Omart, who stands west of Ceril Carnillean's house, just southwest of East Ardougne Castle. Talk to him about crossing the wall - he'll explain that the guards need to be distracted first.
Go to the small watchtower directly north of Omart. Stand in the corner where two spiked fences meet, use the bird feed on either fence, then click your pigeon cage to release the pigeons. This creates the distraction needed.
Return to Omart and talk to him again. He'll call for his assistant Kilron to throw down a rope ladder. Equip your gas mask before climbing over into West Ardougne.
4. Infiltrating the Mourner Headquarters

Head to the Mourners' headquarters in the northeastern corner of West Ardougne. Try to enter, but you'll be denied access because mourners are eating their stew.
Walk around to the northern part of the building. You'll see a gap in the fence - squeeze through it. Pick up the rotten apple on the ground west of the fence opening, then use it on the stew brewing in the cauldron. Squeeze back through the fence.
5. Getting the Medical Gown

Go to Nurse Sarah's house, the large building southwest of the West Ardougne church. Search the cupboard in the northern part to obtain a medical gown. Equip this gown immediately.
Return to the Mourners' headquarters. The medical gown will fool them into letting you inside. Keep this gown equipped for all future visits to the headquarters.
6. Defeating the Mourner and Finding the Distillator

Go upstairs in the headquarters and kill the level 13 Mourner. A key will automatically appear in your inventory after the fight. Use this key on the gate to access the caged room.
Search the third crate from the left on the northern wall to find Elena's distillator. Take it and prepare to leave West Ardougne.
7. Returning to Elena

Exit West Ardougne either by climbing down the manhole in the city center and using the mud pile, or by returning to Kilron at the southeastern corner for help getting back over the wall.
Return to Elena and give her the distillator. She'll provide you with four items: liquid honey, ethenea, sulphuric broline, and a plague sample.
8. Traveling to Rimmington
Make your way to Rimmington using any available teleportation method. The fastest options include House teleport if your house is in Rimmington, Skills necklace to the Crafting Guild, or walking from Falador or Port Sarim.
Find the Chemist in Western Rimmington. You must have the plague sample in your inventory to discuss the quest with him. He'll give you touch paper and explain that Varrock guards are searching for suspicious materials.
9. Using the Errand Runners

Outside the chemist's house, you'll find three NPCs around a campfire. Give each person their specific vial:
Give ethenea to Da Vinci
Give liquid honey to Hops
Give sulphuric broline to Chancy
Getting these assignments wrong will require returning to Elena for new vials.
10. Entering Varrock's Gated Area

Travel to Varrock and obtain a priest gown top and bottom. You can buy these from Thessalia's Fine Clothes for 10 coins or the Fancy Clothes Store near Guidor for 12 coins.
Equip the priest gown and head to the fenced area in southeastern Varrock. The guard will search you when entering, but won't confiscate the plague sample or touch the paper. However, any vials will be taken, so don't bring them through the gate.
11. Retrieving the Vials
Enter the Dancing Donkey Inn inside the gated area. Talk to Hops, Da Vinci, and Chancy to collect the vials they smuggled in. Make sure you have inventory space or you'll need to get new vials from the chemist.
12. Meeting Guidor
Keep your priest gown equipped and go to Guidor's house in the southeastern corner of the gated area. Talk to Guidor and give him the vials and plague sample. Tell him Elena sent a sample for processing.
After analyzing the sample, Guidor will reveal shocking news - there is no plague. The entire situation is a hoax.
13. Completing the Quest
Return to Elena in East Ardougne and report Guidor's findings. She'll suggest speaking with King Lathas about this deception. Go to East Ardougne Castle, located just south of Elena's house.
Climb to the second floor and talk to King Lathas in his throne room. When confronted about the plague hoax, he'll explain his reasons and reveal information about his brother King Tyras and a greater threat from the west.
