Three things push you toward the top of that range. Hard difficulty makes enemies aim better and hit harder, which turns several cover shootouts into repeat attempts. The knife duels reward patience over aggression, so learning the three blade classes costs time. And the drivable map opens up during Chapter 3, which invites detours the story never asks for.
Hangar 13 and 2K were upfront about the scale before launch. 2K president David Ismailer told IGN there's a large audience for compelling stories that don't demand a massive time commitment.

The collectible count is the bulk of it: 50 Mystery Foxes, 50 Newspapers, 23 Photographs and 20 Saints' Cards. The one trophy that demands real commitment is True Soldato, which asks for the whole campaign on Hard and cannot be salvaged by swapping difficulty for the final chapter.
Nothing is lost if you rush the story first. The drivable map opens during Chapter 3 and carries fast travel the story chapters don't offer, so anything you drove past stays reachable. A free update on November 20, 2025 then added Free Ride mode, with repeatable combat, stealth and racing challenges, Dinari to spend on outfits and vehicles, Photo Mode and a Classic difficulty setting. Man of Honor runs about two hours if you play only the two story chapters, and 8 to 10 hours to clear everything in it. The expansion arrived on August 14, 2026 at roughly $10 and needs the base game installed. The Deluxe Edition does not cover it, though the Definitive Edition in the Omertà Collection bundle does.
Design director Josh Zammit laid out the contents on the PlayStation Blog: two story chapters set earlier in Enzo's service to the Torrisi family, with Ennio Salieri handing out work from a Porto Almaro warehouse. The hours come from 68 new collectibles, split into 43 Wanted Posters, 15 Sulfur Trucks and 10 Stunt Jumps, plus the Challenges and Jobs contracts in Free Ride. 2K's Man of Honor FAQ confirms some of that content unlocks through play rather than being available from the start.
Seven trophies come with the expansion and none are missable, so a finished save can go back for them. Added to a full base-game run, you're looking at 23 to 30 hours for the whole package.

Twelve hours reads as thin on a store page and plays very differently in practice. The chapters here are built as set pieces with clean beginnings and ends, so the campaign never hits the sag that a 40-hour crime sandbox has to fight through.
Fifteen to twenty hours for a full clear means the collectibles are dense enough to be worth a Free Ride session. Man of Honor pushing that toward 30 hours changes the value calculation on the base game entirely.
Start on Hard. The combat is the weakest part of the game on any setting. Running the campaign twice to chase True Soldato is a worse use of your evening than eating a few extra deaths the first time through.
