The story takes place in the 1900s in and around the fictional Sicilian town of San Celeste, a location familiar to players of Mafia II. Enzo Favara, voiced by Riccardo Frascari, has spent his early life as an indentured laborer in the sulfur mines, one of the most grueling and dangerous forms of work in early twentieth-century Sicily. Driven by the desire for a better life, Enzo takes the oath and enters the service of Don Torrisi, working his way up through the Torrisi crime family and deeper into the broader network of the Cosa Nostra. The story is built around themes of honor, loyalty, betrayal, and sacrifice, taking direct inspiration from the Sicilian roots of organized crime that the entire Mafia franchise draws from.
Hangar 13 deliberately stepped back from the open-world structure used in Mafia III in favor of a tightly designed linear campaign, citing a return to the focused cinematic storytelling that defined the first two Mafia games. Missions blend third-person shooting, melee combat, stealth, driving period-accurate early twentieth-century vehicles, and horseback riding. The MetaHuman framework from Epic Games was used to create the game's facial animations, and the Nanite and Lumen systems in Unreal Engine 5 power the game's detailed Sicilian environments.
A Free Ride mode was added post-launch in November 2025, allowing players to explore the world outside of the main story with bonus missions and races. A story expansion, Man of Honor, is set to release on August 14, 2026, adding two additional chapters set in the middle of the main campaign.
