The game is a direct successor to BONEWORKS (2019) and builds on its foundation of physics-driven VR interaction. Both games are built around the principle that every object and action in the world should behave physically, from how items are picked up and thrown to how the player's body weight and movement affect balance and momentum.

The story follows an outcast sentenced to death who escapes into a hidden underground research facility called MythOS. From there, players move through a range of experimental game worlds including a main campaign, combat arenas, obstacle courses, tactical trials, and open sandbox areas. Collecting avatars and items across these locations contributes to story progression and unlocks new areas over time.

The avatar system is BONELAB's most distinctive addition over its predecessor. Different avatars have distinct physical properties, including height, weight, and strength. A small, light avatar moves differently and fits through tighter spaces than a large, heavy one. Swapping avatars changes how the player physically interacts with the environment in every context.

Modding support is a central part of the experience. Players can import community-made avatars, levels, weapons, and other items through Mod.io, expanding the available content significantly beyond the base game. The modding community has remained active since launch with a steady stream of new additions.
