The campaign is structured around two timelines. Cold War-era sequences set in the 1980s follow Alex Mason and Frank Woods across locations including Angola, Afghanistan, and Nicaragua, establishing the origin of the central antagonist, Raul Menendez. Near-future sequences set in 2025 follow Alex's son David Mason as he hunts Menendez across a world on the brink of a new global conflict. Multiple endings are available based on cumulative decisions made throughout the campaign.

Kill streaks were replaced with Score Streaks, rewarding objective play and support actions rather than pure kills. Zombies mode returns with new maps and an expanded structure. TranZit is the flagship mode, set across a large open environment split into interconnected areas navigated by a bus. Grief mode adds a competitive layer by pitting two teams of four against each other while both face the zombie threat.
