The game is set in 1622 Japan and follows Tokugawa Takechiyo, a customizable protagonist preparing to assume the role of Shogun. When his younger brother Kunimatsu, consumed by jealousy, aligns with a supernatural force called the Crucible and unleashes yokai across the land, Takechiyo is forced to travel back through multiple historical eras of Japan to stop the plague and undo the damage it has caused.

Combat is built around a dual-style system that lets players switch freely between Samurai and Ninja techniques during fights. The Samurai style retains the series' signature three-stance system covering high, mid, and low positions, each affecting reach, speed, and ki consumption. Players can parry and deflect enemy attacks, and restoring ki through well-timed inputs remains central to the combat loop. The Ninja style offers a faster, more evasive approach built around ranged attacks, shuriken, stealth options, and a Mist clone mechanic for distraction. Each style has access to seven dedicated weapons, giving players 14 to work with across both.

Nioh 3 moves away from the linear mission structure of its predecessors and introduces a semi-open world design spanning several historical eras. Regions reward exploration with hidden bosses, loot, and vertical traversal options. Guardian Spirits return and can be equipped to support combat through dedicated skills and abilities. Nioh 3 holds a Metacritic score of 85 and a Very Positive rating on Steam. Online co-op supports up to three players, and New Game Plus adds further replayability after the campaign.
