Players take the role of Reagents, civilians coerced into Murkoff Corporation's program of experimental brainwashing and mind control. The goal is simple on paper: complete the therapy and earn your freedom. The reality is a series of increasingly brutal trials set across oppressive facilities patrolled by grotesque enemies, requiring stealth, resource management, and cooperation to survive.

The game introduces several systems new to the Outlast series. Up to four players can tackle trials together, each choosing from four distinct classes with upgradeable abilities that complement different playstyles. Night vision goggles return as a core mechanic, with battery management remaining a source of constant tension. A deep customisation system lets players upgrade their character, unlock skills, and personalise their cell between trials.
