The game follows a human child who has fallen into the Underground, a vast realm sealed beneath the Earth by a magical barrier and populated entirely by monsters. To reach the surface, the child must travel through the Underground and find a way past the barrier. The monsters encountered along the way are not simply obstacles. Each has a personality, a history, and the potential to become an ally.

Combat operates on two levels. During enemy attacks, players control a small heart icon and must dodge bullet patterns in a system borrowed from shoot-em-up games. On the player's turn, they can attack, use items, run, or take non-combat actions specific to each enemy. Talking, joking, or reasoning with a monster can change their emotional state and eventually allow the player to spare them entirely.

The game has three broadly distinct playthroughs depending on how every encounter is handled: pacifist, neutral, or genocide. Each produces a different outcome and a different version of the story, with certain routes revealing information that others do not. The game also uses metafictional elements that interact with the player's saved game data across different runs, which has made it one of the most analyzed games of its era.

Undertale was created almost entirely by Toby Fox, who wrote the story, designed the gameplay, and composed the entire soundtrack. The project began as a Kickstarter campaign in 2013.
