The game is set in the year 11945 during the 14th Machine War. Humanity has been driven from Earth by machine lifeforms created by an alien force and now survives on the Moon, sending android soldiers back to the surface in an effort to reclaim the planet. Players begin as 2B, a combat android of the YoRHa unit, alongside her scanner partner 9S. The story unravels across multiple narrative routes, with the second and third playthroughs shifting control to different characters and revealing story content and context that fundamentally recontextualizes events seen in earlier runs.

Combat blends hack-and-slash melee with ranged Pod attacks, dodge-based defense, and occasional shifts into bullet hell sequences during certain encounters. Weapon types include swords, spears, hammers, and combat bracers, and players can equip two weapon sets simultaneously to switch between them on the fly. A Plug-in Chip system replaces a traditional RPG stat structure, letting players slot passive and active abilities directly into their characters and Pods in a customizable grid. Chips cover everything from HUD elements to combat bonuses, and the system extends to removing core interface elements entirely for players willing to accept a harder challenge in exchange.

The game's five endings, labeled A through E, are not simply variations of one conclusion. Routes B, C, D, and E each build on the previous, adding new scenes, new playable sections, and new revelations. Route E in particular requires a decision from the player that permanently alters their save file. The narrative explores themes of existentialism, the value of life, and the reasons people fight, drawing on the philosophical frameworks embedded in the names of its characters.
